Patients with non-paroxysmal atrial fibrillation (AF) may require posterior wall (PW) isolation as part of a comprehensive ablation therapy. The procedure for PW isolation, traditionally using point-by-point radiofrequency (RF) ablation, has also seen the use of various cryoballoon technologies. The feasibility of pulmonary vein isolation, using the Heliostar RF balloon catheter, a novel device from Biosense Webster (CA, USA), was the subject of our study.
Thirty-two consecutive patients with persistent atrial fibrillation, slated for their initial ablation procedure using the Heliostar device, were prospectively enrolled in our study. 96 consecutive persistent AF patients undergoing pulmonary vein (PV) plus pulmonary wall (PW) isolation with a cryoballoon provided procedural data that were compared. Every operator in the study maintained a RF balloon/cryoballoon ratio of 13, thereby minimizing any influence of differing experience levels.
Documented single-shot PV isolation was significantly more common with RF balloon technology (898%) compared to cryoballoon ablation (810%; p=0.002). The attainment of PW isolation involved a similar frequency of balloon applications (114 for RF, 112 for cryoballoon; p=0.016) across groups, but the RF balloon procedure concluded significantly sooner (22872 seconds compared to 1274277 seconds for cryoballoon; p<0.0001). The presence of increased systemic inflammatory cytokines is frequently associated with the manifestation of pathophysiologic events during an infection with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). In order to gain a deeper understanding of the differing cytokine patterns and dynamics within individuals affected by coronavirus disease 19 (COVID-19), and the possible links to mortality, we measured plasma levels of pro-inflammatory and regulatory cytokines in Colombian SARS-CoV-2 survivors and nonsurvivors. The study population comprised individuals with verified COVID-19, individuals with other respiratory conditions requiring hospitalization, and healthy controls. Using either bead-based or enzyme-linked immunosorbent assays, plasma levels of interleukin (IL)-6, tumor necrosis factor (TNF)-alpha, interferon-gamma, IL-10, soluble tumor necrosis factor receptor I (sTNFRI), and transforming growth factor-beta were measured. Simultaneously, hospital records included detailed clinical, laboratory, and tomographic data. In COVID-19 patients, a rise in cytokine levels was observed compared to healthy controls for the majority of the assessed cytokines. The controversy surrounding the role of resection in managing pancreatic cancer with positive peritoneal lavage cytology (CY+) is mirrored by the lack of evidence to support the use of adjuvant chemotherapy (AC) in such patients. This study investigated the predictive power of AC duration on survival in patients with CY+ pancreatic cancer.
Pancreatectomy procedures performed on 482 pancreatic cancer patients between 2006 and 2017 were the subject of a retrospective study. A study comparing overall survival (OS) was performed on patients with CY+ tumors, categorized by the duration of AC.
From the resected patient population, 37 (77%) demonstrated CY+ tumors. 13 patients received adjuvant chemotherapy for more than six months, 15 received it for six months, and 9 received no chemotherapy. A comparison of outcomes in 13 patients with resected CY+ tumors receiving adjuvant chemotherapy for over six months revealed a similar operative success rate to that of 445 patients with resected CY- tumors (median survival times: 430 vs. 336 months, P=0.791). This success, however, was markedly superior to the outcome seen in 15 patients with resected CY+ tumors who received adjuvant chemotherapy for only six months. Statistical significance (P=0.017) was achieved after 166 months of monitoring. The length of AC treatment exceeding six months independently predicted the prognosis of patients with resected CY+tumors, with a hazard ratio of 329 and a p-value of 0.005.
